چکیده

A 72-year-old male was admitted because of dysphagia for solids and liquids. He had undergone a laparoscopic Nissen fundoplication due to erosive esophagitis and hiatal hernia. Three years later, due to symptomatic recurrence of the hernia, surgery with placement of a polypropylene mesh was indicated. After four years he developed epigastric pain and dysphagia for liquids. Gastroduodenoscopy showed a polypropylene mesh occluding almost all the esophageal lumen (Fig. 1). The symptoms worsened, with dysphagia for solids and liquids, with weight loss. First of all, we decided to remove the mesh endoscopically.A first attempt was done by exerting strong traction on the mesh with biopsy forceps without any result. At this point we looked for each one of the blue Prolene 3/0 (Fig. 2A) sutures and cut them with endoscopic scissors FS – 3L1 and FS – 5U 1 (Fig. 2B) (Olympus, Tokyo, Japan). Impact of laparoscopic nissen fundoplication with prosthetic hiatal closure on esophageal body motility: Results of a prospective randomized trial.

HYPOTHESIS Prosthetic crural closure does not adversely influence esophageal body motility. In most patients, postoperative increased dysphagia resolves spontaneously during the first months after surgery. DESIGN Prospective randomized trial. We compared patients who underwent laparoscopic Nissen fundoplication with simple sutured hiatal closure and those who underwent laparoscopic Nissen fun...

Early Laparoscopic Nissen Fundoplication for Recurrent Reflux Esophagitis: A Cost-Effective Alternative to Omeprazole

BACKGROUND Eighty percent of patients treated medically for gastroesophageal reflux disease relapse after treatment. Many of these patients require indefinite treatment with omeprazole to prevent recurrence. Nissen fundoplication has been shown to be effective, safe and cost effective in the management of gastroesophageal reflux disease. We suggest a treatment algorithm, which encourages early ...

Laparoscopic Versus Open Nissen Fundoplication in Infants After Neonatal Laparotomy

BACKGROUND Nissen fundoplication is an effective treatment of gastroesophageal reflux in infants. Laparoscopic procedures after previous laparotomy are technically more challenging. The role of laparoscopic Nissen fundoplication after neonatal laparotomy for diseases unrelated to reflux is poorly described. METHODS This was a retrospective review of open vs laparoscopic Nissen fundoplication ...
